Program Description

New Horizons Wellness Appointment & Inquiry Notifications. When you opt in by checking the SMS consent box on our website contact form, New Horizons Wellness LLC will send you non-marketing text messages about your inquiry, appointment confirmations, and appointment reminders. We may also reply to text messages you send us.

How to Opt In

You opt in by entering your phone number and checking the optional SMS consent box on the contact form at www.myhorizonswellness.com/contact. Consent is not required to submit the form or to receive our services.

Eligibility

You must be 18 years of age or older to opt in to this text messaging program. If you are under 18, a parent or legal guardian must opt in on your behalf.

Message Frequency

Message frequency varies based on your inquiry and appointments.

Costs

Message and data rates may apply. Contact your wireless provider for details about your plan.

How to Opt Out

You can cancel at any time. Reply STOP to any message. After you send STOP, we will send one message confirming you have been unsubscribed, and you will no longer receive text messages from us. To join again, check the SMS consent box on our contact form or text START.
